CAT Team Case Manager

Qualifications

Bachelor’s Degree with major course work in a human services or health education field and one (1) year of full-time experience or a Bachelors’s Degree and two (2) year of full-time experience or equivalent experience working with children who have a serious mental illness.

Responsibilities

This is responsible and specialized case management work in assessing, planning, linking, monitoring, and advocating for a caseload of children/adolescents with serious and persistent mental illness. The incumbent is responsible for assisting clients with their needs/goals through the development of the treatment/service plan and/or aftercare plan. Work also entails responsibility for working with their families or guardians, community resources and advocating for clients by linking them with appropriate services

Forensic Multidisciplinary Team – Therapist (Full Time)

Qualifications

This position must be a Licensed Clinician with at least one year of full-time experience with adults with serious mental illness and co-occurring disorders, and prior experience with individual and group counseling, and with substance abuse interventions. This position is supervised by the Team Leader.

Responsibilities

The FMT model is comprised of a self-contained support team responsible for directly providing or coordinating the majority of treatment, rehabilitation, and support services according to the range of services specified in Section C.2. Services shall be individualized, comply with each individual’s court order, and be provided primarily in out-of-office settings. The FMT model is recovery-oriented, promotes empowerment, and encourages personal responsibility. Guiding principles include participant choice, cultural competence, person-centered planning, stakeholder inclusion, and meaningful input by the individual into their treatment. The FMTs shall promote the safety of the individuals and 2 the community at large while providing oversight and structure to individuals who need community-based services and supports. The FMT shall base services on the principles of Trauma-Informed Care.

Qualifications

High School Diploma/GED -2 years experience in health care field or health care service certification

Responsibilities

Care for individuals with mental or emotional conditions or disabilities, following the instructions of RN or other health practitioners. Monitor patients physical and emotional well-being and report to supervising staff.

Requires an LMHC, LSCW, LMFT, MCAP or CAP; two (2) years experience preferred.

Responsibilities

Provides alcohol and other drug prevention counseling and activities to residents of Brevard County, presents information on the effects of alcohol and other drugs use in various settings, participates in community substance abuse prevention and education planning, and performs related work as required.

Registered Nurse (Full Time & PRN Openings)

Qualifications

Current Florida Registered Nurses License, Associates Degree, 1 year experience preferred and EHR experience preferred.

Responsibilities

The Residential Nurse works under the direction of the program director and clinical services coordinator. The residential nurse must have the ability to make difficult program decisions for a client population with serious and persistent mental illness. Additionally, decisions also involve staffing, safety, security, and infection control issues. The residential nurse must have an understanding of the population served and be able to problem-solve and educate employees in each of the Community Support departments. Work is reviewed through meetings with the program director and clinical services coordinator, evaluation of intervention decisions, compliance with state and federal rules, regulations, and agency policies and procedures.
