This is responsible and specialized case management work in assessing, planning, linking, monitoring, and advocating for a caseload of adults with serious and persistent mental illness. The incumbent is responsible for assisting clients with their needs/goals through the development of the treatment/service plan and/or aftercare plan. Work also entails responsibility for working with their families or guardians, community resources and advocating for clients by linking them with appropriate services.

Job Duties
  • Provide case management/therapeutic mentoring services to an assigned group of clients
  • Meet with assigned clients on a weekly basis or as indicated by treatment planning
  • Act as a liaison and consult with school systems, community agencies, and families to maintain coordination of the treatment process
  • Advocate for families’ rights and needs
  • Refer adult and families for additional services as identified
  • Meet expectations regarding documentation deadlines and utilization trends
  • Preparation of social skill building activities for each client
  • Assist with assessing safety issues that may have an impact on the service
  • Provide transportation, respite and tutoring services as assigned
  • Bachelor’s degree in human services or related field
  • Six (6) months experience required one (1) year of experience preferred
  • Valid driver’s license, with clean driving record
